If your loved one is unable to care for themself during the day, our Adult Day Health Services program can help. We provide professional care for people facing frailty, disability, chronic illness, or other challenges that prevent them from caring for themselves.

Our experienced staff provide healthcare, rehabilitative therapies, recreational activities, and social services for adults 18 and over. Your loved one will find a welcoming, safe, and therapeutic environment that helps them feel supported and connected.

Services We Offer

Our Adult Day Health Services program offers a variety of treatments and services to help participants feel their best, enjoy a better quality of life, and maintain their independence. Our services include:

  • Skilled nursing: Nursing care is available as needed from registered nurses (RNs), licensed vocational nurses (LVNs) and certified nursing assistants (CNAs).
  • Physical therapy: Physical therapists provide exercises and therapies to help build strength, balance, and flexibility.
  • Occupational therapy: Occupational therapists work with participants to improve skills needed in daily life.
  • Speech therapy: Speech therapists help with skills to rehabilitate speech and improve communication.
  • Behavioral and emotional healthcare: Clinical psychologists and therapists provide support and referrals for mental health concerns.
  • Nutritional counseling: Dietitians assist with meal planning and nutritional support.
  • Recreational activities: Recreational therapists lead a variety of engaging activities to provide physical and mental stimulation.
  • Social services assistance: Social workers provide support and resources for financial concerns, transportation, housing, and more.
  • Hot lunch and snacks: Hot, nutritious meals and snacks help meet participants’ dietary needs.
  • Transportation: Transportation to and from the center is available for participants.
  • Services in English, Spanish, and Tagalog: Our team provides services in a variety of languages to help ensure all participants feel included and have their needs met. 

What to Expect

We understand the challenges that come with caring for someone who needs help with daily living. Our Adult Day Health Services is here to offer quality care and support to your loved one. Participants are treated with respect and compassion, no matter their condition. 

If you or a loved one is interested in attending our program, we will first request a History & Physical (H&P) form to be completed and signed by the prospective participant’s primary care physician. When we receive the form from the physician, we will begin the admission process. This includes scheduling assessments to determine which services will be most appropriate. 

First Visit

During the first visit, we’ll work with the participant to make sure we understand and can meet their needs. Plan to spend about four hours at the center. After an initial assessment with our nursing staff, the participant will have a hot lunch and snacks. 

Please bring the following to the first visit:

  • A change of clothes
  • All medications being taken in their original packaging
  • Copies of paperwork requested

Diagnosis & Treatment

Depending on the participant’s needs, they may also have assessments scheduled with other providers, such as a physical therapist, activity coordinator, dietitian, or social worker. Most participants have two additional assessment visits.

Once assessments are complete, we will develop a care plan tailored to the participant’s needs. 

Follow-Up Visits

Starting with the fourth visit to the center, participants receive specific services as outlined in their care plan. We will regularly review and adjust care to ensure participants are getting the services they need.

Referrals and Insurance

Your loved one will need a referral to attend Adult Day Health Services. If they do not have a referral, we will connect them with a referring primary care provider. 

We accept Inland Empire Health Plan (IEHP) coverage or Medi-Cal insurance. If your loved one has other insurance, please contact us at 909-558-6198 for help.
